The FHA is a comprehensive questionnaire that provides education and asks in depth questions about your gynecological, obstetrical, reproductive health and life style factors in order to build your fertility profile. Once completed, you can review your fertility health assessment with a member of our concierge team who will provide you with personalized insights, recommendations, and guidance based on your unique situation.

For our limited timed introductory offer, you are entitled to one 15-minute check-in each month. You can schedule these at your convenience to discuss any questions or concerns you may have.

If you require additional support, you can book extra time with our team for personalized care. This service is available for a small additional fee. Simply reply to our emails or contact our Concierge Team to schedule.
